Transportation Requests
We are happy to provide you with transportation to your medical appointments. The days for transportation are Tuesdays and Thursdays. Please complete a trip request form located at the front lobby, one day prior to your scheduled appointment. We ask that you make your appointments between 8:30 am to 4:30 pm on Tuesday and Thursdays.
Be in the lobby at least 30 minutes prior to your scheduled appointment time. After your appointment, you may call the office to be picked up.
We will make every effort possible to accommodate all appointments, but on rare occasions if there are too many appointments, we may need to ask you to reschedule.

If you would like to attend shopping trips, meet at the front lobby and be ready to go 10 minutes prior to the scheduled departure. Bus schedules vary, so check your activities calendar. We require a minimum of four participants.
